NodumForms offers 13 block types — 11 for collecting answers and 2 for displaying static content between questions.
Text inputs
Short Text — A single-line text field. Use for names, titles, URLs, or any brief text. Shows a smart placeholder based on the question (e.g. "John Smith" for name fields).
Long Text — A multi-line text area. Use for messages, descriptions, feedback, or any longer-form text input.
Choice blocks
Multiple Choice — Radio buttons for single selection. Add as many options as you need. Options can be reordered by dragging.
Single Select — Button-style option cards shown all at once, one pick only. A visually distinct alternative to Multiple Choice when you want each option to feel like a prominent action.
Checkbox — Checkboxes for multiple selection ("select all that apply"). Use this when respondents can pick more than one option. Options can be reordered by dragging.
Dropdown — A compact select menu that collapses into a single line. Best when you have many options and want to save vertical space. Respondents click to expand and pick one option.
Rating
Rating — A number scale (default 1–5, configurable up to 10). Respondents click a number to rate. Great for satisfaction scores, NPS, and quality assessments.
Binary
Yes / No — Two large buttons for a simple binary choice. Use for consent questions, confirmations, or any yes/no scenario.
Contact fields
Email — An email input with built-in format validation. Submissions are rejected if the email format is invalid.
Phone — A phone number input. Accepts any format — no strict validation, so international numbers work fine.
Special blocks
File Upload — Let respondents attach files. Supported formats: PDF, DOC, DOCX, PNG, JPG, JPEG, GIF, WEBP. Default limit: up to 5 files per submission, 5MB each. Both limits are tweakable per block in the block settings panel.
Display blocks
Not every block collects an answer. Two are for context, not input:
Display text — A heading + body paragraph rendered inline between questions. Useful for section dividers, instructions, or legal copy. Text alignment is configurable.
Display image — Drop a hosted image into the form (URL + alt text). Great for branding banners, diagrams, or option swatches inside long surveys.
