The Donation Form template helps your charity or community group collect donation pledges and donor details in one place. Supporters tell you how much they would like to give, whether it is a one-off or regular gift, and share their contact details so you can follow up. Every pledge arrives in your responses dashboard, giving you a clear view of supporter interest and the information you need to turn goodwill into completed gifts.
Adapt the form in the NodumForms drag-and-drop builder to fit your appeal. Use a single select or dropdown for suggested pledge amounts, short text for a custom amount, email and phone fields for donor contact details, and a single select to indicate a one-off or monthly gift. Add a yes/no field for Gift Aid eligibility and a long text field for a message of support. Apply your charity branding through the Styles panel, including your logo and a cover image, so the form reflects your cause and builds trust with donors at the moment they decide to give.
It is important to be clear that this form collects pledges and donor details rather than taking payment; NodumForms does not process card payments, so the actual donation is completed in your own payment or fundraising platform. Use the dashboard to see pledges arrive with the live response count, read messages of support in the Individual view, and export everything to CSV so you can follow up by email, set up regular gifts or import donors into your finance and CRM tools. Delete responses on request to respect supporters' data preferences.

- Does the form take card payments?
- No. It collects donation pledges and donor details, not payments. NodumForms does not process cards, so complete the actual donation in your own payment or fundraising platform after exporting the pledge.
- Can I capture Gift Aid details?
- Yes. Add a yes/no field for Gift Aid eligibility and any declaration text your charity uses, so you can record which pledges qualify and follow up appropriately when collecting the gift.
- How do I follow up with donors?
- All pledges appear in your dashboard. Export the donor list to CSV to email supporters, set up regular gifts in your payment platform, or import contacts into your CRM for future appeals.
