Overview
The Appointment Request template lets patients ask for an appointment online and share the times that suit them best. They provide their contact details, the reason for the appointment and a few preferred dates or time slots. Each request arrives in your responses dashboard, where your team reviews it against the diary and confirms a slot directly with the patient, replacing back-and-forth phone calls during busy reception hours.
Set the form up in the NodumForms drag-and-drop builder to match how your practice schedules. Use short text and email fields for the patient's name and contact details, a single select or dropdown for preferred day or time of day, short text fields for two or three preferred dates, and a long text field for the reason for the appointment. Add a yes/no field to flag whether the request is urgent. Apply your branding through the Styles panel so the form feels like an extension of your practice and gives patients confidence their request will be handled properly.
It is important to be clear that this captures an appointment request, not a live booking: there is no connected calendar, so a slot is only confirmed once your staff respond. This keeps you in control of the diary and avoids double bookings. Share the form by link in reminder emails or embed it on your website so patients can request a time whenever it is convenient.
